A well demarcated jaw with a distinct neck line have defined beauty thoroughout the ages . Traditionally women have smooth jaw lines that come to an apex at the chin forming an inverted triangle, which is offset by a sleek long neck; men have strong angular jaw lines and pronounced chins, which contrast with the vertical angle of a muscular neck. With age, however, these shapes give way to more androgynous round and square shaped faces.

Over time, the underlying muscles in the face and neck weaken and sag, laugh lines become pronounced, excess skin and fat accumulates in the lower face forming jowls,masking the jawline, creating fullness under the chin, and a "wattle" along the neck. The midline of the neck also may take on a sunken appearance with vertical bands on either side. Gravity literally pulls your face downward robbing your face of its youthful uplifted appearance. Not only age, but genetic predisposition can also commonly lead to form fatty deposits in the neck.

Midfacelift
Midfacelift (Endoscopic) (cheek lift), a minimally invasive surgery, elevates sagging skin and cheek fat, reducing smile lines and relieving lower eyelid depression.The endoscopic procedure has several advantages to its surgical counterparts including faster recovery, inconspicuous scarring, and fewer side effects. Although patients with minimal to moderate skin excess are best suited for the endoscopic procedures, this procedure can be performed alone or in combination with a Rhytidectomy (face lift).

Facelift
Facelift (rhytidectomy) smoothes and tightens the skin and underlying muscles of the lower face reducing jowls and wrinkles. Face lifts done as early as the mid 40s can significantly slow the aging process in the lower region of the face. Because excess skin and fat must be removed to correct these particular signs of aging, the endoscopic technique is not used.

  • A facelift usually takes two to five hours depending on the extent of the procedure.
  • The incision line is hidden in the hairline or scalp starting at the temples and continuing around and behind each ear. The incisions are made without clipping or shaving hair. Incisions are carefully camouflaged by placing them in hair or near facial borders. The most obvious scar is usually behind the mid to top of the ear.
  • The skin is raised from the temples, cheeks, and neck and the underlying connective tissue (muscle and necessary fat) is lifted and repositioned. Excess fat and skin are removed.
  • Dr. Goldman redrapes the skin over the new underlying structure and closes the incisions with fine stitches and small metal clips.
  • A dressing is placed to keep the incisions clean and dry and also apply presure to help with natural healing.

Necklift and Neck liposuction
Necklift tightens the skin and muscles of the neck and makes the muscles less prone to further sagging as you age.The best neck lift is a face lift but for a small number of patients, particularly younger patients, a smaller procedure can be done under the chin to help the neck lines. This usually involves Liposuction or Platysmal band tightening.

Neck liposuction removes the fat that has collected in the lower face and underneath the chin. Liposuction of the neck is an appropriate method for men and women of all ages.

  • Dr. Goldman makes a small incision under the chin. Through the incision, the edges of the platysmal muscles (the muscles running down the neck) are located, drawn together, and stitched at the midline.
  • The back portion of these sheet-like muscles is tightened and fixed at the back of the neck on either side forming a strong sling of muscle that supports the entire neck and jaw.
  • Liposuction, if necessary, can be done through the same incision.
  • The incision is stitched up and a soft dressing is placed around the head to protect the incisions.

Post-Op information - Facelift, Necklift and Neck liposuction
After the procedure is over, patients may experience some numbness, bruising, swelling or discomfort at the incision sight. This is normal and will usually pass with time. Incision site pain is usually minimal, but can be controlled with medication if necessary. Most people use pain medicine the first few days, particularly at night and then switch to extra strength Tylenol (nothing with aspirin in it). Keep your head elevated and as still as possible for a couple of days after surgery to reduce swelling. You should also be up and about after two days, but you should take it easy. Avoid vigorous activity for at least 3 weeks.

On the first morning after surgery the dressing is removed and changed. You are taught how to care for it then. Stitches are removed after about five days, although the stitches and metal clips in the hairline could be left in a few days longer. Bruising may persist for two to three weeks, but may be covered with make-up.

Because this is a very invasive surgery you'll probably have less energy for the first two weeks. By the third week, you'll look and feel much better. Most patients are back at work in about two weeks after surgery. Above all, get plenty of rest and allow your body to heal.
